How do you know if you or a loved one could be at risk of falling?

If you are concerned about the risk of falls for yourself or a loved one, you should talk to your doctor about your concerns and consider scheduling a fall prevention evaluation.

If you are unsure, use the following questions based on the Missouri Alliance for Home Care Fall Risk Assessment Tool to help identify fall risk. 

Read the following statements, and count how many are true for you. 

  • I am 65 years old or older. 

  • I have three or more current medical diagnoses. 

  • I regularly take four or more medications.

  • I have fallen one or more times in the last three months. 

  • I have difficulty getting to the bathroom on time. 

  • I have difficulty seeing some things in my home and/or I have been diagnosed with a visual impairment. 

  • I frequently need help with tasks around the house; I have difficulty and/or pain with walking, getting out of a chair, and getting out of bed; I am worried about falling during my daily routine; I have impaired sensation or numbness in my feet; I have difficulty coordinating movements. (If one or more of these statements are true for you, count "one point" regardless of how many are true.)

  • Some parts of my home have insufficient lighting, items that are hard to reach, and/or uneven floor/ground.

  • My pain stops me from doing things that I want to do. 

  • I have trouble remembering things.
